Deadline for full payment of registration fee and completion of the Team Roster Form is April 16th, 2010. Please ensure all required information is completed. This will guarantee your team’s place in the tournament.
Entry into the tournament is on a “first come, first served” basis.
Players can play for only one team in the tournament.
All players will be required to sign waivers of liability and provide photo ID (proof of age; if requested) prior to being allowed to compete.
“On-site” check in is from 10:00 am to 5:00 pm on Friday, April 30, 2010. Each team must check-in on Friday to claim tournament passes, game schedule, player programs etc.
Registration Fee is $500 per team. Teams withdrawing from competition prior to April 1, 2010, will be refunded one-half (1/2) of the registration fee. Teams withdrawing after April 1, 2010, will not receive registration fee refund.
The Registration Fee includes insurance coverage.
To hold your spot download the Team Registration Form . This will ONLY hold a spot for you until which time your Team Roster and Registration Fee is received (deadline April 16, 2010).
Please download and print the Team Roster Form and the Team Waiver Form (right click and “save as”). Once the Team Roster Form has been completed (deadline April 16th, 2010), mail it along with your Registration Fee to:

Specific to this event, we have arranged for additional coverage for 1,000 participants and officials, as follows:
- Accidental Death and Dismemberment – $10,000
- Dental Injury – $1,000
- Taxi Fare – $50
- Fracture – $1,000
- Rehabilitation – $5,000
- Tutorial – $2,000
